This 20 credit module, Advancing Practice in Long Term Conditions, available at level 6 (UZWSVQ-20-3) or level 7 (UZWSVR-20-M), will introduce you to the concept and definitions of long-term conditions.

Long term conditions are more prevalent in older people and in more deprived groups. Treatment and care for people with a long-term condition are complex and multi-faceted, many people live with more than one long term condition increasing, this is expected to rise. There is a rising demand for prevention and management of multi-morbidity rather than a single disease process.

This online module will help you to demonstrate knowledge and understanding of concepts related to the anatomy, physiology and clinical reasoning that supports recognition of long term conditions.

On successful completion of this module, you will be able to:

• evaluate the evidence of case management to formulate an action plan for the treatment of long-term conditions.
• work collaboratively with other healthcare professionals and illustrate leadership to ensure high quality, effective care and treatment for patients with long-term conditions.
• identify, discuss and evaluate policy and professional issues related to the complex management of long-term conditions.

Content

In this online module, you will examine some of the key areas that a long-term condition can affect a person's life. It will take a holistic approach to assessment and introduce you to different ways of managing an individual's care. This will include health and social care, and examine the importance of self-management. You will also look at key policy drivers to inform your practice and learning.

Learning and Teaching

A variety of approaches will be used which may include expert speakers from practice, flipped classroom strategies, case study review, online interactive teaching, and lecture-style classroom teaching.

Assessment

The assessment for this module comprises of one assessment task; a presentation (20 minutes maximum). The presentation will be delivered online and consist of 10 minutes (maximum) duration and up to 10 minutes for questions.

Throughout taught sessions, learners will contribute to case studies for critical discussion, enabling them to prepare for the summative assessment. Optional online sessions with facilitation will be provided between the taught sessions and assessment date to help build an assignment plan and practice presentation skills.
